INCIDENT REPORT FORM
SAFETY - CHILD WELFARE, SECURITY & SAFETY, INCIDENT & FIRST AID REPORTING
This form is a CREW form so we can keep a register with accurate information of any and all matters related to child injury/illness that occurs during Crew activities, first aid, security, and child welfare. This generates an email that is submitted to the Principals. If it is urgent, please also ensure you text, call or messenger the Principals. The content is confidential and will only be used where appropriate or legally required to do so to ensure the safe and secure operation of all Crew activities.
If it’s a matter of immediate safety concern or medical emergency, CALL 000. If appropriate, then contact student/person’s emergency contacts as well as the Principals.

DEBRIEFING AFTER AN INCIDENT - FIRST AID, CHILD SAFETY, NEAR MISS, SECURITY, ACCIDENT
In the event of an injury or incident that is more than a minor event, it is important to ensure you ‘the first aider’ or tutor are feeling ok. Red Cross recommends considering the following questions:
How did I react?
Did I stay calm?
What else could have I done?
How can I improve my skills?
What other equipment might have been appropriate?
Do I need to discuss this with a trusted manager or colleague, or other health or mental health professional?
Please keep the communication lines open with Leiz 0419146634 or Allan 0418146634, and discuss any serious matter or even concern about a potentially serious matter with Leiz or Allan, as well as have a debrief conversation with one of them. If you are not feeling OK after an incident or injury, we encourage you to talk with your Lead Tutor or one of the Co-Principals (Leiz or Allan) or a health professional such as a GP or psychologist.
